Parliamentary report: ‘Human error’ led to Dec 8 chopper crash killing Gen. Bipin Rawat

India’s first Chief of Defence Staff, Gen Bipin Rawat, died in a Mi-17 V5 helicopter crash near Coonoor, Tamil Nadu, on December 8, 2021. The crash also claimed the lives of his wife, Madhulika Rawat, and several armed forces personnel. A parliamentary report now attributes the accident to “human error” by the aircrew.
The Standing Committee on Defence tabled its report in Parliament on Tuesday. It highlighted 34 Indian Air Force (IAF) aircraft crashes during the 13th Defence Plan period, spanning multiple years. The report noted nine crashes in 2021-22 and 11 in 2018-19.
The data specifically referenced the Mi-17 crash on December 8, 2021. It labeled the cause as “HE(A)”—human error by the aircrew. This finding aligns with inquiries conducted into similar incidents during the same period.
The Defence Ministry informed the committee about 34 inquiries held to investigate these crashes. These inquiries recommended improvements across various domains, including processes, training, equipment, and operations. According to the ministry, the Chief of Air Staff’s remarks on these recommendations are mandatory for implementation.
While most measures have already been actioned, a few remain under implementation. The ministry assured that these steps aim to prevent future accidents and improve safety standards.
This revelation underscores the critical need for enhanced training and robust safety protocols within the armed forces. It also brings a tragic reminder of the loss of one of India’s most distinguished military leaders.
